Use this checklist to help wrap up important loose ends after divorce.1. Obtain a copy of my certified divorce decree. Make extra copies.
2. Change my name on my:
____ Driver’s license, automobile title and registration
____ Social security card
____ IRS records
____ Automobile insurance policy
____ Life, health, homeowner’s and disability insurance policies
____ Employer’s records
____ Credit cards
____ Bank, brokerage and investment accounts
____ Professional licenses
____ Title to real property
____ Utility bills
3. Apply for COBRA health insurance, if necessary.
4. If a Qualified Domestic Relations Order (QDRO) is required per the divorce decree, follow up until it is
provided to appropriate banks, brokerages, investment houses, pension plans, 401k administrators)
5. If the divorce decree requires a quitclaim or warranty deed, follow up until executed and recorded.
6. If the divorce decree calls for the transfer of title to automobiles, motorcycles or boats, prepare, sign,
and deliver the necessary documents to complete the transfer.
7. Remove my name, or my ex-husband’s name, from any joint accounts or mortgages.
8. Close any joint credit accounts.
9. Open a new bank account and credit card account.
10. Set up direct deposit or income withholding for child support, spousal support or alimony payments.
11. Write and execute new will, trust, and powers of attorney.
12. Change the beneficiaries on my life insurance, 401k, pension and IRA accounts.
13. Update mailing address and forward mail.
14. Order a new copy of my credit report after joint accounts are closed.
15. If applicable, keep records of all child support payments made or received, all medical visits, insurance
claims, uninsured medical expenses and co-pays paid.
16. Other: Anything else your divorce judgment or court requires of you.
